Use the main entrance facing Ponca Place.  There is parking in front of and next to the main entrance. 

Our office is on the second floor, accessible by elevator.  Ask at the Reception Desk to be directed to our office.

    Go west on Baseline Road

    Turn left onto Mohawk Drive (second traffic light)

    Turn left onto Pawnee Drive

    Turn left onto Ponca Place

    Frasier Meadows is on your left

    Louisville, CO 80027

Located in the Balfour Retirement Community - The Residences Independent Living building.

Use the entrance on the east side of the building.  There is designated parking near the clinic entrance. 

   

From South Boulder Road:

    Turn North on Courtesy Road/Route 42

    Turn right onto Hecla Drive

    Take your second right to the east side entrance of The Residences
